The VA Roseburg Healthcare System (VARHS) is seeking to hire a highly skilled and experienced Nurse Practitioner licensed as an independent attending practitioner for the Consult Care Center (CCC) provides consultative services across the healthcare spectrum and collaborates with clinicians on the inpatient, outpatient, and patient/residential units. The position is located at the VA Roseburg Healthcare System main campus. The CCC APRN is organizationally aligned under the Acute Care and Community Living Center Nursing Service, reporting directly to the Nurse Manager for Patient Flow. The candidate is responsible for demonstrating leadership, experience, and creative approaches to the clinical management of consults for client care within VA and non-VA care.
This includes organizing, inputting, and coordinating the clinical components for VA and non-VA services in response to individual healthcare needs along the illness and care continuum and in multiple settings. The goals are to center services around the patient, to foster a coordinated consult management process, and maximize efficiency and cost-effective use of healthcare resources. The CCC APRN will provide clinical guidance, assistance, and leadership to the consult management team. The CCC APRN will guide the consult process through the various treatment modalities either within VARHS, within other VA facilities, or within community resources. The incumbent facilitates interdisciplinary collaboration to resolve complex patient care issues for patients receiving treatments and serves as a key contact person in the coordination of specialty care consult patients who are receiving care from both VA and non-VA care resources. This includes at a minimum:
Triage of consults (in terms of priority and appropriateness for care within VARHS, within VISN, within VHA, or external)
Identification of patients appropriate for purchased care services (based on eligibility and availability of needed services with VARHS or another VA)
Coordination of clinical care with other VA providers and/or non-VA providers
Collaboration with Fee Basis services to maintain fiscal responsibility and efficiency
Integration of the patient back into the VA system when purchased care episode is completed
Monday - Friday (Day Shift) with some weekends (alternating)

QUALIFICATIONS
YOU MUST:
Be a U.S. citizenship.
Be a graduate of a school of professional nursing and have a Master's degree in Nursing. Education program must have been approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of course work equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
Have current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e. Puerto Rico) of the U.S. or in the District of Columbia AND must be licensed or otherwise recognized as a Nurse Practitioner in a State. (NOTE: This position requires the incumbent to work as a licensed independent practitioner. Applicant must possess a license from a State granting independent practice or, if selected, must be willing to obtain a license from a State granting independent practice upon hire.) Must be certified as an Acute Care, Adult Care, or Family Nurse Practitioner by the American Nurses Association or other nationally recognized certifying body. Must meet the physical requirements of the position. (Successful completion of a pre-employment physical will be required prior to appointment to this position.)
This position is one with direct patient care responsibilities. Candidate selected must be proficient in written and spoken English.
The nurse practitioner selected for this position functions at an Advanced Practice Nurses (APN) who are masters degree-prepared registered nurses who also possess advanced clinical certification. They function within a scope of practice (SOP), rather than a position description, commensurate with their training, experience, and licensure. An APN functions autonomously within her or his own defined SOP in a variety of settings, such as hospital inpatient, outpatient clinics, nursing home, domiciliary, or patient's home. Competitive levels should be established using the basic CL criteria, including APN pay levels: Nurse I, II, III, IV, and V.
Nurse Practitioners and Clinical Nurse Specialists . On and after March 17, 2009 , registered nurses appointed or otherwise moving into these assignments must meet and maintain the following additional qualifications. This includes employees appointed before March 17, 2009 , who obtain such qualifications on or after March 17, 2009.
A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected.

Physical Requirements:
This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, kneeling, pulling, and pushing. Transferring patients and objects may be required. The candidate may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The candidate will be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The candidate will be working closely with others and may have protracted or irregular hours of work on occasion.
